Advocate Hospice was started to promote excellent patient care at the end of life by Dr. Kevin Jackson and his spouse Barbara Jackson, who strive to provide outstanding and innovative patient care delivery.
Dr. and Mrs. Jackson previously owned and operated multiple medical agencies across Maricopa County since 2001, all sharing the goal of improving patient care in the Valley. Advocate Hospice is their latest creation with the philosophy of providing hospice care to eligible patients, sustaining the highest possible quality of life until death, and using their experience and expertise to deliver unrivaled hospice care.

We believe that life should conclude naturally with a focus on quality of life in addition to the length of life. Also important to maintaining physical wellbeing is emotional and spiritual support which is shared with our patients and their families and caregivers.
Our team of professionals support the patient, their families and their loved ones, with the core belief being that each one of us has the right to die pain free and with dignity. Advocate Hospice promises to provide all of the support necessary to families' loved ones to accomplish this goal.

Our Mission

Advocate Hospice and Palliative Care LLC will advocate to maintain the dignity of all our patients with life limiting illnesses that have chosen to seek comfort care and symptom management, and to their families, and those who care for them.


Services

We provide the following services throughout the majority of Maricopa county:

Physician Services
Meeting with patients and their families in order to assess whether or not hospice is best for the patient, in addition to diagnosing and treating symptoms.
Nursing Care
Routine visits with the patient to treat symptoms and administer medical care.
Medical Equipment
Related to terminal illness, which may include: hospital bed, specialty mattress, wheelchair, walker, oxygen.
Medications for Symptom Control or Pain Relief
All medications related to terminal illness for pain and symptom management prescribed by the patient's physician.
Hospice Aide Services
Housekeeping and assistance with hygiene duties: bathing, dressing, hair and oral care, feeding, and ensuring patients' comfort.
Social Worker Services
Helping patients and their families to understand and plan for end-of-life care, including access to support networks for overcoming emotional, familial, and financial struggles.
Chaplain Services
Offering spiritual guidance for patients and their families at the end-of-life in addition to accommodating religious practices for patients when requested.
Dietary Counseling
Guides patients to form healthy eating habits in addition to formulating diet plans with nutritious foods.
Bereavement and Loss Counseling
Offers patients and their families bereavement support for grief and loss for up to 13 months after a patient's death.
Volunteer Services
Providing patients with companionship through readings, playing games, performing errands, and offering support to patients and their families.
